Event Details:

MPJamie Schmale's office will be hosting a Passport Clinic to help you and your family renew your passports.

Please note of the following things to bring and have ready when you arrive:

1. Identification

Original birth certificate* Canadian citizenship card and/or a Canadian Passport

*(if you are applying for a child’s first passport, the birth certificate must include proof of parentage)

**(current or most recently expired passport if it is within 15 years of the issue date)

2. Secondary ID: Required only for Adult General Applications 

- Photocopy of BOTH SIDES of driver’s licence or photo health card—signed by your guarantor

- ID must be issued by federal or provincial government. It must contain: name, date of birth, photo, and signature. You may use two pieces of ID to satisfy the requirements if needed

- If you have a change of name, supporting documents such as a marriage certificate or divorce decree may be required by Passport Canada

3. Passport Fees

Credit card, money order, or certified cheque payable to the Receiver General of Canada

$120 Adult 5-year

$160 Adult 10-year

$57 Child (0-15 years of age)

4. References 

Names, full mailing addresses, and phone numbers of two people NOT RELATED TO YOU who have known you for at least 2 years

5. Guarantor: Adult General and Child 

A guarantor is a Canadian citizen, 18 years of age or older in possession of a valid Canadian passport who has known you for at least two (2) years. A family member or person residing at the same address as you may be your guarantor, provided he or she meets the above requirements.

*Please note: A guarantor is not required when completing an Adult Simplified Renewal Application. My office will not be able to provide a guarantor.

6. Photos

There will be a photographer on site. You are not required to use this service; however, pictures are required as part of the application process.

The on-site photographer will accept direct payment.
